Inspection Reports Summary Info An editor-reviewed summary of the themes and findings across this facility's recent inspection reports.

The most recent inspection on August 28, 2025, found a deficiency related to failure to ensure referral and follow-up for a resident’s seizure medication due to communication and cost issues. Earlier inspections showed a pattern of deficiencies involving health care referrals, medication administration errors, and medication storage security. Prior reports also noted issues with staffing levels, infection control practices during COVID-19, and safety concerns such as unsecured medication carts and exit door alarms. Complaint investigations included substantiated findings related to infection control and multiple resident care and facility maintenance issues. The facility’s deficiencies have persisted over time with some recurring themes, and recent inspections indicate ongoing challenges with health care coordination and medication management.

KK
My family member has been a resident for 1.5 years. The cost has risen from $5000 to $5600/month.She has no way of securing her room. When out of the room , anyone can walk in and take whatever they want. This includes other residents as well as staff. Items include shoes, clothes, household items and lastly, a set of sheets. It does no good to speak with the director or floor staff. No action is taken. The bed is not being made correctly even after speaking with the director. The top sheet is not tucked in and ends up balled up. This past Friday, the sheets were removed moved from the bed by staff for laundering. They were not returned nor the bed made. She asked 2 of the floor staff, but no action was taken. I later found out that she had been left to sleep on top of the mattress cover for 2 - TWO nights! Unacceptable.I also understand that one of her evening medications was allowed to run out. For 2 - TWO nights she did not get the medication or sleep. UNACCEPTABLE!One thing that she really liked was the food-the variety, great desserts and quality. In the last few months, the chef left. The person brought on to make their food is a much lower level chef. The quality has decreased significantly . She now complains about the food and often skips meals.I also had one of the floor staff to speak with me about her behavior. She had become friends with another resident. They spent a lot of time together. Sometimes laying on the bed to watch tv. This staff member told me that she had found them in bed together, naked, having sex. She told me it was the most “disgusting” thing she had enter seen. I am not sure that I want someone with such a unaccepting attitude/opinion taking care of my elder.I don’t understand why this expensive facility allows such things to happen. I hope DSS will continue to monitor their operations.
