The following deficiencies were found during the on-site compliance inspection and investigation of complaint AZ00199018, AZ00201053, and AZ00203885 conducted on February 14, 2024.
Findings
The inspection found four deficiencies related to documentation of vaccination notifications, written medical determinations for residents confined to bed or chair, notification of emergency contacts and primary care providers after incidents, and documentation of preventive actions following incidents.

A manager failed to ensure resident medical records contained dated documentation of notification of influenza and pneumonia vaccination availability for four of nine residents reviewed, posing a potential illness risk.
R9-10-814 — The manager failed to ensure the facility obtained a written medical determination at least every six months that residents confined to a bed or chair could have their needs met by the facility, for two of three residents reviewed.
The manager failed to ensure caregivers immediately notified residents' emergency contacts and primary care providers when incidents requiring medical services occurred, for two residents reviewed, posing a health and safety risk.
