A health care initial licensure survey was conducted to assess compliance with licensing requirements.

Findings
Eighteen deficiencies were identified related to criminal history checks, relocation agreements, admission and service agreements, toxic chemical storage, medication management, resident care records, fire safety inspections, food preparation, nursing assessments, and resident rights.

.216.02 Written Agreement: six of eight sampled residents did not have admission agreements created and signed by all responsible parties on or before the change of ownership as confirmed by the administrator.
.260.07 Toxic Chemicals: toxic chemicals including isopropyl alcohol, Comet deodorizing cleanser, and Spectracite Bug Stop were stored in an unlocked closet accessible to cognitively impaired residents; staff secured the chemicals after being informed.

.310.01.g Medication Distribution System: the facility did not ensure all ordered PRN medications were available; Resident #9 and Resident #2 lacked several medications which were confirmed missing by the medication technician.
.310.02 Discontinued and Expired Prescriptions: medication destruction logs lacked required components such as method of destruction, full names of persons destroying medications, witnesses, and documentation of some destroyed medications as confirmed by the facility nurse.
.320 Negotiated Service Agreement (NSA) Requirements: after change of ownership, NSAs for all sampled residents were not reviewed or updated, did not clearly identify residents, nor adequately describe services to be provided.
.330.04.c.vii Resident Care Records: pre-admission assessments were not documented for residents admitted on or after the change of ownership; the nurse admitted to conducting but not documenting assessments.
.330.08.b Additional Resident Records: the administrator did not document complaints received from residents and family members regarding theft and cold food; no system was in place to track complaints.
.330.13 Personnel Records: staff records lacked required documentation including orientation, infection control, specialized training, and current CPR/first aid certifications as confirmed by the administrator.

.410 Requirements for Emergency Actions and Fire Drills: no fire drills had been conducted since licensing on June 1, 2025, failing to meet bi-monthly drill requirements; the administrator was unable to provide documentation.
.460.01 Food Preparation: residents were provided food that was often cold due to uncovered plated food left on an unheated cart and open steam table lids; complaints were received and the steam table was not functioning properly.
