Visit Reason
On-site compliance (annual) inspection of an Assisted Living Home conducted on September 2, 2025.
Findings
The inspection found nine deficiencies related to tuberculosis control, staff English proficiency, personnel records, resident rights including restraint use and privacy, medication administration documentation, food storage, and hazardous material storage. Plans of correction were provided for all deficiencies.

R9-10-113 — The facility failed to provide annual tuberculosis training and education to one of three personnel sampled, posing a risk due to lack of TB surveillance instruction.
R9-10-803 — The governing authority failed to ensure a caregiver who could read, write, understand, and communicate in English was on the premises, as a caregiver who did not speak English was scheduled daily and designated as manager.
R9-10-806 — The manager failed to ensure employees provided documentation of freedom from infectious tuberculosis for three personnel sampled, missing required TB risk assessments and symptom screenings.
R9-10-806 — The manager failed to maintain a personnel record for employee E1 and lacked required documentation for E3, including caregiver certification and verification of qualifications.
R9-10-810 — The manager failed to ensure a resident was not subjected to restraints, as bedrails were observed in the upright position contrary to facility policy prohibiting their use.

R9-10-811 — The manager failed to ensure medication administration records included the name and signature of the individual administering medication for two residents, with untrained staff administering and documenting medications improperly.
R9-10-818 — The manager failed to ensure food requiring refrigeration was maintained at 41°F or below, as opened condiments requiring refrigeration were found stored in the pantry.
R9-10-820 — The manager failed to ensure poisonous or toxic materials were stored in locked areas inaccessible to residents, with cleaning supplies found unsecured in multiple locations.
