No contact was made to the attending physician until one month later. State investigators determined that a one-month delay in medical attention for a potential UTI constituted noncompliance with regulations.

The facility opened with massive physical plant violations, then demonstrated wholesale failure to implement basic admission, medication, and resident protection procedures.
