- The Collinfield House underwent a comprehensive inspection on April 17, 2019, resulting in two state standard violations. The Health Code deficiencies cited included the failure to train all staff in reporting abuse and neglect prior to their assuming job responsibilities, which was corrected by May 1, 2019, and the failure to post the DADS telephone hotline number to report suspected abuse, neglect, or exploitation, which was corrected on April 18, 2019. No Life Safety Code deficiencies were cited during this inspection. The facility had no enforcement actions taken against it as a result of these violations.
