Visit Reason
Initial certification visit to determine compliance with certification rules for an Assisted Living Program.
Findings
Three regulatory insufficiencies were cited related to missing required policies, medication administration errors, and failure to develop and sign initial service plans before occupancy agreements.
Violations (3)
481-67.2 Program Policies and Procedures: The program failed to have policies addressing sexual relationships between tenants and staff and between tenants with advanced dementia, and lacked a policy for extraordinary lifesaving measures such as CPR.
481-67.5(2)f(4) Medications: The program failed to administer the correct dose of Vitamin D3 to Tenant #4 and administered Fiber Well Gummies not included on the tenant's medication list.
481-69.26(2) Service Plans: The program failed to ensure initial service plans were developed and signed before occupancy agreements for three tenants, with service plans signed days after occupancy agreements.
