Medication assistance and care coordination: Four records were intermittently missing staff initials or documentation indicating observation of residents’ medication administration, and there were 860 emergency call response times over ten minutes in April 2023, October 2023, and January 2024.

Incident reporting: The Residence filed 3 incident reports more than 24 hours after the occurrence of the incidents during the period reviewed.

Emergency call response: Missing e-call response time documentation and lack of effective monitoring to the emergency response system, including missing documentation of hourly safety checks (repeat finding).

Quality assurance program: Missing or incomplete components of the Quality Assurance and Performance Improvement requirements, including missing documentation of follow-up actions and staff responsible, and unsafe medication administration observations (repeat finding).

Resident records: Correspondence Logs did not communicate clear and legible information necessary for continuity of care, and required disclosure forms were missing signatures in five resident records (repeat finding).

Information given to residents: Inconsistent documentation with all required disclosures and Resident Rights and Services disclosure.

Recordkeeping: Personnel records did not contain signed and dated job descriptions for three staff members.

Staff health screening: Missing documentation confirming receipt or declination of seasonal influenza vaccine and completed tuberculosis screening for four staff members.

Reports filed with the state: Missing documentation of the 2022 Aggregate Data Report and required notification of Assisted Living Residence Manager change within ten business days.
