The visit was triggered by complaints #118147-C, #118081-C, #118079-C, and #117958-C regarding tenant treatment and care issues.
Findings
The program failed to ensure tenants were treated with dignity and respect, failed to retain communication logs for three years, did not fully evaluate or update service plans and conduct nurse reviews following significant changes in tenant conditions, and failed to include tenant preferences in service plans.
