Union Court St. Charles is an 86-bed nonprofit assisted living facility at 302 Fulton Street in St. Charles, Saginaw County, Michigan, operated by Baruch SLS, Inc. The facility failed to complete and submit timely incident reports with required information.

An investigation on December 2, 2022, substantiated that Resident A had an unwitnessed fall with unexplained injuries. The facility failed to check alarms and assist with toileting every two hours as required. Hospital transfer was delayed, and the family contacted before a physician. Documentation shows the facility lacked an organized program ensuring resident protection.

An October 2023 administrative review found substantial regulatory compliance. No enforcement actions or fines are recorded. The license.

It is imperative to request clarification on current protocols for timely emergency response, resident alarm monitoring, incident reporting procedures and timeframes, medication administration oversight, and resident protection programs.
