- September 26, 2024 annual inspection found five deficiencies including failure to maintain hot water temperatures, nonworking call bell system, insufficient SCU staffing, incorrect diet, and delayed emergency response causing a resident death.
- Prior construction surveys from December 13, 2023 and March 28, 2024 cited ongoing physical plant hazards such as unsecured oxygen bottles, malfunctioning electromagnetic locks, plumbing issues, and fire safety equipment failures.
- June 19, 2024 follow-up survey confirmed all previously noted construction deficiencies were corrected with no further action required.
