Visit Reason
Biennial state compliance review of the assisted living residence by the Massachusetts Executive Office of Aging & Independence (AGE) on October 17, 2024.

Findings

Emergency call response: In the months of December 2022, June 2023, and August 2024 there were multiple emergency call response times over the 10 minutes required by the Residence policy.
Resident screening and assessment · Resident care plans · Bed rail safety: Documentation was missing for residents' risk for elopement, assessment findings for behaviors presenting risk, timely service plan reassessments, and physical/occupational therapist assessments for bed rail use (repeat finding).
Quality assurance program: Documentation of the date of review was missing for calendar years 2022 and 2023, and medication assistance observations revealed missing resident identification and a loose pill in storage (repeat finding).
Incident reporting: The Residence filed 26 incident reports greater than 24 hours after the occurrence of the incident during the period reviewed (repeat finding).
Staff training: Documentation was missing for annual training needs assessments, introductory visits, and semiannual PCA skills evaluations (repeat finding).
