Sommerset Neighborhood is a 126-bed not-for-profit, faith-based assisted living and memory care community (License AL1402) at 1601 Southwest 119th Street, Oklahoma City, Cleveland County, Oklahoma 73170. Administered by Jessica Guillory, it was founded in 1998 and operates with a mission emphasizing competency, compassion, and ethical behavior. The community offers varying care levels allowing residents to remain in place as needs change.
Sommerset maintains a strong compliance record with improvement following a single deficiency cluster. Thirteen Oklahoma State Department of Health inspections between February 2019 and November 2025 identified five deficiencies, averaging 0.8 per year, 84% better than Oklahoma’s average of 4.9. All five deficiencies occurred May 9, 2023, during relicensure and involved kitchen cleanliness (improper dish storage, unlabeled food products), incomplete admission assessments, lack of RN or physician coordination on assessments, improper blood pressure medication administration, and inaccurate PRN medication documentation. A plan of correction was submitted and accepted; compliance was confirmed June 8, 2023. All inspections since June 2023, including February 2025 renewal, November 2025 relicensure, and November 2025 specialized memory care disclosure, identified zero deficiencies. Complaint investigation June 2019 found zero deficiencies with all allegations (abuse policy implementation, care per contract) unsubstantiated. No fines, immediate jeopardy findings, or enforcement actions have been imposed. Renewal license effective November 30, 2025 through November 30, 2028.
The 126-bed facility comprises studio, suite, and one-bedroom units in a single-story building. Personalized assistance with activities of daily living, medication management, housekeeping, laundry, and chef-prepared meals with attention to dietary preferences and restrictions are provided. The specialized 20-bed Alzheimer’s and dementia care unit offers staff trained in behavioral management, communication techniques, cognitive stimulation, and therapeutic activities with 2 hours of structured programming daily. CNAs and licensed nurses provide care. Occupancy is 82% (103 occupied beds). Walk Score 48; facility is 10.1 miles from Oklahoma City center. BBB Rating: A-(Not Accredited). The facility participates in Oklahoma ADvantage Waiver and Personal Care Program for Medicaid-eligible residents.
The May 2023 deficiency cluster, centered on kitchen operations, assessments, and medication documentation, represents the only significant finding across seven years of inspection data. The 2.5-year clean record and swift corrective action since June 2023 reflect responsive management. The specialized memory care designation with dedicated staffing and training indicates commitment to dementia care expertise.
