Fire safety compliance fell short: smoke alarms were not operational (with low battery warnings observed), a fire safety and evacuation plan was not in place, required staff training had not been completed, and evacuation drills had not been conducted twice per shift as regulation requires. Physical conditions failed maintenance standards: a clogged floor drain, leaking faucet, and unsecured electrical outlet without a cover were documented.

Additionally, the registered nurse had not completed required training and competency testing of unlicensed personnel across all mandated skill areas.

Operating Monday through Friday from 9 a.m. to 5 p.m., with stated 24/7 supervision, it admits residents age 18 and older.
