Info below is compiled from the UT Dept. of Health & Human Services (DHHS), senior community websites & trusted data sources such as Walk Score & BBB.

Communities are listed from highest-ranked to lowest-ranked based on our methodology.

Care Types in This Table AL (Assisted Living): Housing with help for daily activities like bathing, dressing, and medication, without 24-hour skilled nursing. MC (Memory Care): Secured, specialized care for people living with Alzheimer's or dementia. RC (Respite Care): Short-term temporary care that gives family caregivers a break. IL (Independent Living): Community living with dining, activities, and transportation for active seniors who need little personal care. ADC (Adult Day Care): Daytime supervision, health monitoring, and social activities for seniors who live at home.
City and state of the facility. Used for mapping and state-average comparisons.
Licensed bed capacity (maximum allowed). Not the same as how many beds are currently filled.
Indicates whether residents are allowed to have pets in the facility.
Third-party neighborhood walkability score (0–100). Convenience/context metric, not a healthcare quality measure.
Description of bed or unit types offered (e.g. assisted living, memory care), as reported in state or facility disclosures.
Number of years the company has been in operation.
A 0–100 score of a facility's regulatory inspection record, shown as a letter and compared with other facilities of the same kind in the same state. This is a proprietary Assisted Living Magazine grade. It is not a government rating. A dash means we have no inspection data for that facility.
Count of regulatory inspections in the reporting window. Higher volume can reflect more oversight visits, complaint-driven surveys, or routine licensure cycles.
How many inspections had one or more deficiencies recorded. Compare to total inspections to see how often problems were found on visit.
Total count of deficiencies (cited regulatory issues) recorded in the selected reporting period. Read alongside inspection counts for context.
The average number of deficiencies this facility receives each time it's inspected, calculated as total deficiencies ÷ total inspections. A lower number suggests more consistent compliance with safety and care standards.

Overview of Lotus Park Senior Living

West Haven’s Lotus Park Senior Living holds a 54-bed license as an Assisted Living Facility Type II, maintaining state certification since May 2012 (13 years of operation). Located at 2639 West 3520 South in Weber County, the community operates three residential models: Memory Care in a secured unit, Assisted Living, and Independent Living, serving exclusively private-pay residents without Medicare or Medicaid certification.

Resident life includes amenities spanning a fitness center, library, spa, walking trails, pool, and on-site salon. The dining program is chef-prepared with attention to nutritional balance and individual dietary accommodation. Licensed nurses and certified nursing aides staff the facility 24/7, and residents meet through an active Resident Council. The neighborhood is car-dependent (Walk Score 15) with limited nearby walkable options; the facility sits 7.1 miles from downtown West Haven.

State inspection activity from 2024 onward shows five inspections total, with two identifying deficiencies.

The May 6, 2024 routine inspection uncovered 16 violations, which is double the Utah assisted living average, spanning staff readiness, legal compliance, and operational systems.

Specific findings: one employee lacked orientation documentation; two new hires were missing required one-on-one training; four staff had incomplete annual and competency training. Personnel files showed three employees not entered into the state DACS system prior to hire, seven criminal background forms missing, and three employees fingerprinted past the 15-day deadline. The administrator had not delegated successor authority in writing. Two exit lights were inoperative and fire doors in the memory care unit had unsealed half- to three-quarter-inch gaps. The emergency plan lacked protocols for missing residents and mass casualty events; personnel assignments, recruitment procedures, and care continuity under emergencies were undefined.

Resident records revealed three assessments without memory documentation and two assessments inaccurate to current resident status. One resident eviction lacked a signed 30-day notice and another lacked documented ombudsman notification. The facility’s washing machine and clothes dryer were unavailable for resident use.

No complaint investigations or prior inspection reports were available for historical comparison.

The facility structures itself around private-pay memory care and assisted living in a rural Utah environment, though the May 2024 inspection documented wide-ranging administrative, personnel, and emergency-readiness deficiencies requiring correction.

A house physician is available at all times, giving residents access to physician care through the community. Residents can use the beauty salon for personal services and the private family dining room for meals with visiting family members. Chris Hermansen serves as the director, and the smaller setting may be a good fit for someone who prefers a community with fewer residents.

Overview of Quail Meadow Assisted Living and Memory Care

Quail Meadow Assisted Living and Memory Care is in North Ogden, Utah. It has been open for 12 years. There is room for 53 residents, and the community offers two kinds of care: assisted living and memory care. Of all the rooms, 21 are studio-style. Each one has a walk-in closet, its own bathroom, and its own temperature control.

The Walk Score here is 17. That is low and it means most people need a car to reach stores or run errands. Staff take care of housekeeping and laundry. Meals are homemade, and they are made to fit what each resident needs to eat. The community also offers scheduled rides. That way, residents can get to a doctor’s visit or an errand even if family can’t drive them that day.

The showers are walk-in style and have safety features. This helps residents who need extra support moving around. Utah’s Department of Health and Human Services checks on the building. This is done through a group called Health Facility Licensing. Past visits have looked at things like how medicine is handled, fire safety tools, emergency plans, staff training, and how well records are kept.

Quail Meadow offers two types of care, home-cooked meals, and safer bathrooms built for residents who need more support. All of this makes it a solid fit for families looking for both assisted living and memory care in one place.

Frequently Asked Questions about Assisted Living in Utah

What's the difference between assisted living and memory care in Utah?

Assisted living in Utah supports residents with daily activities (bathing, dressing, medication management) while preserving independence. Memory care is a specialized form of assisted living for residents living with Alzheimer's or dementia, and adds 24/7 secured environments, dementia-trained staff, and structured routines designed to reduce confusion and wandering.

What's the difference between assisted living and a nursing home in Utah?

Assisted living in Utah is a residential model focused on housing, hospitality, and help with daily activities. Nursing homes (skilled nursing facilities) provide 24/7 medical care from licensed nurses for residents with significant health needs, and are regulated more strictly under both state and federal CMS rules.

Does Utah Medicaid cover assisted living?

Utah Medicaid does not directly pay room-and-board for assisted living, but most states (including Utah) offer Home and Community-Based Services (HCBS) waivers that can offset the cost of care services delivered inside a licensed community. Eligibility, waitlists, and covered services vary — check directly with the state Medicaid agency.

What is assisted living?

Assisted living combines private or semi-private apartments with help on activities of daily living (meals, bathing, dressing, medication management) and a social calendar — for residents who need some support but do not need 24/7 medical care.

How many assisted living facilities are listed on this page?

This page features 157 assisted living facilities in Utah. Use the filters and comparison tools above to compare ratings, amenities, and pricing.

How do I choose the right assisted living facility in Utah?

Start by matching the level of care offered to the resident's current and anticipated needs, then compare licensing status, staff-to-resident ratios, recent inspection results, and pricing. Tour at least two or three communities in Utah, talk to current residents and families, and confirm what is included in the base rate versus billed as add-on services.

What should I look for when visiting assisted living facilities in Utah?

Pay attention to staff interactions with residents, cleanliness and odor, food quality at meal times, the activity calendar, and how questions about pricing and care plans are answered. Ask to see the most recent state inspection report, the move-out / level-of-care-change policy, and a sample monthly bill that lists every fee.
