Village at Oakwood is a 110-bed assisted living and independent living community (License AL5534) at 817 Southwest 59th Street, Oklahoma City, Oklahoma County, Oklahoma 73109. Administered by Fidelia Oglesby and managed by John Wilson, it has operated for 19 years since 2010. The community is designed as an affordable option, partnering with government programs to provide financial assistance, offering personalized care plans and apartment-style living with residents’ own furnishings.

The facility maintains an exceptional compliance record across six years of inspection data with the Oklahoma State Department of Health. Ten inspections between May 2019 and April 2025 identified a single deficiency, averaging 0.2 per year, which is 96% better than Oklahoma’s average of 4.9. The most recent April 23, 2025 complaint investigation documented one deficiency: the facility failed to submit an incident report to the state within one business day for one resident involving unwanted sexual advances between residents. Executive Director Fidelia Oglesby admitted to not reporting this allegation to the state agency initially. The same complaint investigation alleged sexual and psychosocial abuse, physical and verbal abuse, involuntary seclusion, medication administration errors, over sedation, and meal quality issues. All allegations except the reporting failure were either not substantiated or unsupported by documented findings. Prior complaint investigations (June 2023, January 2023) found zero deficiencies with all allegations unsubstantiated. Renewal inspections and a COVID-19 routine survey all showed zero deficiencies. No fines, immediate jeopardy findings, or license suspensions have been imposed. A renewal license was issued February 4, 2025, effective May 1, 2025.

The 110-bed facility comprises one-bedroom apartments in a two-story building with 545 square feet per unit. Residents furnish with personal belongings. Twenty-five full-time staff provide support. Services include personalized assistance with meals, medication management, personal care, housekeeping, transportation, and recreational activities. All-inclusive room and board covers rent, utilities, maintenance, and meals. Fitness and recreation support resident engagement. Current occupancy is 73% (80 of 110 beds), reflecting recent market demand decline. Walk Score 67 indicates moderate walkability; facility is 5 miles from Oklahoma City center. BBB Rating: A+ (Not Accredited). The facility participates in Oklahoma’s ADvantage Waiver and Personal Care Program for Medicaid-eligible residents.

The April 2025 deficiency involved procedural failure to report to state authorities rather than substantiated allegations. The isolated reporting lapse amid otherwise clean compliance history and corrected through plan of correction represent a management oversight that was addressed. The six-year inspection pattern supports effective operational management.
