Administered by Fidelia Oglesby and managed by John Wilson, it has operated for 19 years since 2010. The community is designed as an affordable option, partnering with government programs to provide financial assistance, offering personalized care plans and apartment-style living with residents’ own furnishings.
The facility maintains an exceptional compliance record across six years of inspection data with the Oklahoma State Department of Health. Ten inspections between May 2019 and April 2025 identified a single deficiency, averaging 0.2 per year, which is 96% better than Oklahoma’s average of 4.9. The most recent April 23, 2025 complaint investigation documented one deficiency: the facility failed to submit an incident report to the state within one business day for one resident involving unwanted sexual advances between residents. Executive Director Fidelia Oglesby admitted to not reporting this allegation to the state agency initially. The same complaint investigation alleged sexual and psychosocial abuse, physical and verbal abuse, involuntary seclusion, medication administration errors, over sedation, and meal quality issues. All allegations except the reporting failure were either not substantiated or unsupported by documented findings. Prior complaint investigations (June 2023, January 2023) found zero deficiencies with all allegations unsubstantiated. Renewal inspections and a COVID-19 routine survey all showed zero deficiencies. No fines, immediate jeopardy findings, or license suspensions have been imposed. A renewal license was issued February 4, 2025, effective May 1, 2025.
