Sakura Gardens of Los Angeles is indeed a vibrant community that is warm and welcoming to its residents, making families of residents feel secure in knowing that their loved ones are getting the best care they can get.
The luxurious senior living community houses state-of-the-art amenities such as their beautiful common areas, dining areas, bedrooms, and fitness center, indoor atrium, computer or media center, movie room, and meeting room; these are among the thoughtfully designed features included in the building that provide help for seniors to find everything they may need. The community encourages an active lifestyle for their residents so as to cultivate their mental and physical abilities– arts and crafts with art classes, golf course or putting green, Live Well programs, day trips and outings, and live dance or theatre programs.
The Village at Northridge is a 194-bed residential care community at 9222 Corbin Ave, Northridge, Los Angeles County, California, operated by Northridge Subtenant LLC and SRG Management LLC under administrator Thomas Rekowski. The community offers assisted living and independent living, serving seniors with active recreational and residential services in a mid-rise building with one- and two-bedroom units ranging from 584 to 1,258 square feet. The facility licensed in June 2015 expanded its capacity from 125 to 194 beds in 2022 and maintains an 85-percent occupancy rate, above the California assisted living average of 72.8 percent.
The regulatory landscape at The Village at Northridge warrants careful consideration. The California Department of Social Services Community Care Licensing Division has documented a pattern of substantive deficiencies requiring direct family inquiry before placement. Most critically, a May 21, 2025 Type A violation found the facility failed to maintain auditory alert systems on exterior doors and perimeter gates for residents at risk of elopement; thiere was a direct safety breach when one resident with documented dementia or cognitive decline departed the premises unsupervised across the street, requiring 911 intervention. This violation points to inadequate supervision protocols for vulnerable populations and remains the most urgent compliance concern in the current record.
Cleanliness and sanitation deficiencies appear repeatedly: substantiated complaints in 2021 documented cockroaches and mold in resident rooms alongside improper chemical storage accessible to residents. A July 31, 2025 deficiency observed stained carpeting, unclean bedsheets on the floor, unclean toilets, and kitchen clutter in a resident’s quarters. These findings suggest inconsistent housekeeping standards and quality oversight. Resident dignity breaches emerged from a substantiated complaint (August 15, 2024) in which a staff member yelled at and humiliated a resident regarding a pet incident in front of other staff; the investigation confirmed this conduct through multiple staff interviews, and corrective action was completed by August 19, 2024.
Fall prevention represents a critical emerging gap. A substantiated complaint documented that one resident experienced 18 fall incidents during 2024, yet the resident’s service plan acknowledged fall risk without specifying actions to prevent future falls. A September 26, 2025 inspection found two fall incidents with no documentation of unusual incident reports; these are regulatory violations in required reporting. A December 30, 2024 deficiency noted the care plan did not sufficiently address fall risk management.
Additional substantive findings include a delayed response (from February 24 to April 3, 2025) in releasing resident records to an authorized representative; a violation of prompt-access requirements.
Amenities reported include swimming pool, spa, fitness centers, library, beauty services, transportation, dining, and recreational programming. Walk Score 61 indicates moderate walkability. Room sizes and flexible meal planning are offered. The facility accepts pets and identifies itself as a luxury senior living option.
A pending death investigation commenced December 5, 2025, and remains ongoing as of that date.
