The September 16, 2025, complaint investigation issued two initial comment citations with zero corrective action documented. The February 26, 2025, complaint investigation revealed fourteen deficiencies in notice requirements, activities, activity professional qualifications, safety hazards, dialysis, RN coverage, staffing information, medication storage, food sanitation, employee background checks, nursing services, and administrative rules; many were corrected by March 22, 2025, though some persisted through April 3, 2025. A March 8, 2024, complaint investigation found five deficiencies in bed hold policy notification and quality of care spanning bowel care, wound care, nutrition, comprehensive assessment, medication regimen review, RN coverage, medication errors, records accuracy, and training; some were corrected after initial visit but not at follow-up. An October 20, 2023, complaint investigation documented twenty deficiencies covering notification of physician-relevant changes, safe environment, comprehensive and accurate assessment, wound care, nutrition monitoring, RN coverage, drug regimen review, medication errors at or above the 5 percent threshold, resident records, and nurse aide training; many were corrected by November 12, 2023, but remained uncorrected by December 5, 2023.

Six focused infection control surveys from October through December 2022 repeatedly cited failure to report complete COVID-19 information to the CDC’s National Health Safety Network, with citations uncorrected throughout. Medication errors meeting or exceeding the 5 percent error rate flagged in the October 2023 inspection point to systemic gaps in pharmaceutical safety oversight.

Complaint investigations were unsubstantiated. No fines or license actions are recorded.

The community provides skilled nursing, rehabilitation, memory care, and specialty services with 24/7 nursing, therapy, daily living assistance, nutritional support, and activities. Private pay is required; Medicaid and Medicare are not accepted.

Families should ask about corrective actions taken following the October 2023 inspection, regarding medication errors, RN coverage, comprehensive assessments, and wound care, and request verification on tour that quality-of-care measures are in place and functioning.
