Licensed under Washington Department of Social and Health Services, Residential Care Services, the facility comprises studio and one-bedroom units (297 to 610 square feet) in a three-story building. Jamie Bravato serves as administrator.

The facility permits pets and provides licensed nurses and CNAs, rehabilitation services, respite care, housekeeping, and transportation. Amenities include a theater, salon and barbershop, fitness center, library, bistro, activity and art studio, private dining room, and outdoor garden terrace. A full-service restaurant provides meals; a bistro serves snacks. Current occupancy is seventy-three percent.

Washington Department of Social and Health Services, Residential Care Services conducted five inspections between March 2023 and August 2025, identifying nineteen total deficiencies. The facility averaged 6.3 deficiencies per year, 39 percent below the state average of 10.3.

The most recent inspection on August 20, 2025, found zero deficiencies. Prior citations addressed elopement prevention policy failures (one resident eloped and sustained injury on elbow), incomplete staff background checks (one staff member worked 501 days without valid background check, exposing thirty-eight to thirty-nine residents to risk), food menu documentation gaps, tuberculosis screening failures, bed rail safety issues, and fire safety documentation deficiencies including firewall and sprinkler system inspections, fire alarm testing, smoke detector and carbon monoxide detector testing, and emergency lighting and generator documentation.

One complaint investigation substantiated an elopement incident; another addressed failure to implement a Respiratory Protection Program during Covid-19 exposures (four residents and one staff member tested positive). No enforcement actions such as fines or license suspensions were documented.

Located in the Hawthorne Hills neighborhood 6.2 miles from Seattle’s city center with a walk score of seventy-eight, the facility is 0.44 miles from the nearest hospital.

Washington Medicaid programs including COPES, Community First Choice, and Washington Family Caregiver Support Program provide coverage for qualifying residents.

Families should request complete documentation of corrective actions for all cited deficiencies covering fire safety, staff background checks, elopement prevention, and Respiratory Protection Program implementation.
