The dining program is chef-prepared with attention to nutritional balance and individual dietary accommodation. Licensed nurses and certified nursing aides staff the facility 24/7, and residents meet through an active Resident Council. The neighborhood is car-dependent (Walk Score 15) with limited nearby walkable options; the facility sits 7.1 miles from downtown West Haven.

State inspection activity from 2024 onward shows five inspections total, with two identifying deficiencies.

The May 6, 2024 routine inspection uncovered 16 violations, which is double the Utah assisted living average, spanning staff readiness, legal compliance, and operational systems.

Specific findings: one employee lacked orientation documentation; two new hires were missing required one-on-one training; four staff had incomplete annual and competency training. Personnel files showed three employees not entered into the state DACS system prior to hire, seven criminal background forms missing, and three employees fingerprinted past the 15-day deadline. Resident records revealed three assessments without memory documentation and two assessments inaccurate to current resident status. One resident eviction lacked a signed 30-day notice and another lacked documented ombudsman notification. The facility’s washing machine and clothes dryer were unavailable for resident use.

No complaint investigations or prior inspection reports were available for historical comparison.

The facility structures itself around private-pay memory care and assisted living in a rural Utah environment, though the May 2024 inspection documented wide-ranging administrative, personnel, and emergency-readiness deficiencies requiring correction.
