On-site complaint investigation at Faith Hospice, a Hospice Service Agency, conducted 13 May 2024.

Complaint Details
Deficiencies were found during the unannounced on-site State Complaint survey conducted on May 13, 2024.
Findings
The inspection found six deficiencies related to admission assessments, care plan authentication, and discharge orders. The facility failed to ensure proper psychological assessments, timely physician signatures on care plans, and physician-written discharge orders for multiple patients.
