Emergency call response: During the months of August 2024 and October 2025, there were 42 e-call response times over the 10-minute limit (repeat finding).

Resident care plans: Two records were missing documentation of individualized enrichment activities provided to the Residents in the SCR (repeat finding).

Quality assurance program: Documentation of the date the audit was completed was missing for the 2023 and 2024 calendar years, and multiple medication administration and storage issues were observed (repeat finding).

Emergency drills · Incident reporting: Documentation of a third shift elopement drill was missing for 2024 and a third shift fire drill was missing for 2023; the Residence filed 17 incident reports greater than 24 hours after the occurrence of the incident or accident (repeat finding).

Controlled medication handling: Documentation of CS count procedure by staff was inconsistent, with improper documentation of count errors and declination of CS medications not recorded at administration (repeat finding).

Resident records: Both SCR Correspondence Logs did not consistently document all significant or pertinent information (repeat finding).

Staff training: Documentation confirming the completion of biannual skills evaluations was missing from three Personal Care staff records for 2024 and 2025 (repeat finding).
