- August 30, 2017 survey found 13 deficiencies including obstructed corridors, unsecured oxygen cylinders, missing fire extinguisher tags, compromised fire-rated walls, and multiple fire door latch failures.
- May 23, 2023 follow-up found two deficiencies related to delayed egress door signage and non-working exhaust fans but no new major safety hazards were reported.
