Overview of The Reserve at Towne Lake

The Reserve at Towne Lake is a 50-bed memory care and rehabilitation community in Woodstock, Georgia. The facility offers customized memory care programming, on-site health services, rehabilitation, and respite care with daily meals, housekeeping, laundry, salon services, and personal assistance. Amenities include visiting clinical services, fitness activities, musical performances, arts and crafts, games, and church services.

Over two years, the facility recorded 4.5 deficiencies per year, 8% better than Georgia’s average of 4.9. An August 2019 complaint investigation revealed insufficient staffing for memory care residents, incomplete quarterly care plan reviews, and medication administration errors. A November 2019 complaint investigation substantiated failure to protect residents from aggressive behavior by other residents, inadequate family notification about condition changes, and failure to investigate and report serious injuries and abuse. An earlier 2017 inspection found no violations.

Prospective residents with cognitive impairment or behavioral concerns should review detailed inspection reports before placement. The facility’s documented care deficiencies require careful family consideration. Safety protocols for resident-to-resident interactions warrant particular attention.
