BROOKDALE CLUB HILL underwent a comprehensive inspection on June 02, 2022, resulting in six state standard violations. The facility was cited for two Health Code deficiencies: failing to provide a locked area for all medications, which was corrected by September 9, 2021. The remaining four deficiencies were classified under the Life Safety Code and included issues with egress routes, exit signs, emergency lighting, and smoke compartmentation. All of these Life Safety Code violations were corrected by July 14, 2022. No enforcement actions were found prior to a subsequent administrative penalty on July 16, 2025, which assessed a fine of $2450 for failing to ensure each resident was free from abuse, neglect, and exploitation.
