The Community Care Licensing Division conducted three unannounced investigations at the Cogir of Folsom residential facility (Facility Number 345002909). The 2024 visit, initiated by a complaint received in January 2024, revealed a severe lapse in resident supervision that resulted in the death of resident R1. Staff failed to intervene during an altercation in which resident R2, known to have a history of aggression, forcibly shoved R1, leading to a spinal fracture, infection, and subsequent homicide‑designated death. The department found the allegation substantiated, imposed an initial civil penalty of $500, and documented additional penalties under review. Deficiencies were cited under Section 87468.2(a)(4) of the California Health and Safety Code, requiring the facility to conduct an audit of residents with combative behaviors, integrate aggression protocols into care plans, and provide comprehensive intervention training for all staff. The plan of correction deadline was set for December 24 2024, with a second deadline of January 23 2025 for corrective action implementation.
