Inspection Report Summary for Cornerstone Court

The most recent state inspection report for Cornerstone Court, dated October 27, 2023, identified deficiencies related primarily to resident care and safety. Inspectors found issues with negotiated service agreements, including failure to include proper catheter care instructions for one resident, and failure to update service agreements after changes in residents' conditions for another. The facility also failed to ensure timely tuberculosis testing for one staff member and did not complete safety assessments for medical devices and smoking for several residents. Environmental concerns included unsecured hazardous supplies accessible to residents. The facility submitted plans to correct these deficiencies by November 20, 2023. Earlier, a follow-up inspection on December 18, 2023, found no deficiencies, indicating improvement. Fire safety inspections from May and July 2025 showed some issues with testing and maintenance of sprinkler system backflow preventers, but these were addressed by July 1, 2025. The most recent state inspection on August 14, 2025, found medication management deficiencies, including failure to notify physicians when residents refused medications and failure to communicate with providers when medication orders were held, placing residents at risk. The facility has committed to corrective actions with deadlines in late August and September 2025. There were no fines, immediate jeopardy findings, or license suspensions noted in the reports. The trend shows some improvement after the 2023 deficiencies but recent medication management issues in 2025 suggest ongoing challenges in that area.
