Visit Reason
Unannounced routine inspection to assess compliance with assisted living facility regulations.
Findings
The inspection identified 12 rule noncompliances across various regulatory areas including administrator duties, personnel training, resident assessments, service plans, food services, housekeeping, emergency preparedness, and medication administration.
Deficiencies (12)
R432-270-8(1)(a-p) The facility does not have a designate in writing to act as administrator when the administrator is unavailable for immediate contact.
R432-270-9(7)(a)-(f) One employee did not have documented approved CORE competency training.
R432-270-11(8)(a)-(g) The admission agreement does not contain a notice that the department has the authority to examine resident records to determine compliance with licensing requirements.
R432-270-13(1) One resident assessment was not completed before admission and one resident assessment was not completed within 6 months.
