Visit Reason
The inspection was a pre-inspection visit conducted to assess compliance with licensing requirements before issuing a license to the facility.
Findings
The facility was found to be serving clients without a license and was non-compliant with multiple licensing and program requirements including background checks, policy submissions, physical facility standards, and program administrative rules.
