Visit Reason
The inspection was a pre-inspection visit conducted to assess compliance with licensing requirements before issuing a license to the facility.

Findings
The facility was found to be serving clients without a license and was non-compliant with multiple licensing and program requirements including background checks, policy submissions, physical facility standards, and program administrative rules. Several areas such as congregate care services and seclusion rooms were not provided or not compliant.
