Facility failed to maintain sanitary kitchen conditions placing residents at risk of illness and contaminated food. Multiple food service and dietary deficiencies noted with corrective actions implemented.

Facility failed to reorder medication timely causing missed doses and failed to provide comprehensive pharmaceutical and nursing services. Some deficiencies corrected after revisit, others remained uncorrected.

Multiple deficiencies including failure to maintain safe and clean environment, inadequate grievance process, failure to provide appropriate ADL care, insufficient activity programs, poor quality of care, pressure ulcer prevention, mobility maintenance, trauma informed care, behavioral health services, and dietary services. Many deficiencies were corrected after follow-up visits, some remained uncorrected.

Facility failed to ensure cognitively impaired residents were free from accident hazards resulting in a resident sustaining a second-degree burn. Deficient practice was past non-compliance and corrected by 11/16/23.

Facility failed to follow resident's plan of care for falls resulting in a resident fall with brain injury. Deficient practice was past non-compliance and corrected by 3/22/24.
