GRAND BROOK MEMORY CARE OF ALLEN underwent a comprehensive inspection on January 02, 2023, resulting in six state standard violations. Of these, four were classified as Health Code deficiencies and two as Life Safety Code deficiencies. The Health Code issues included the failure to assess or develop service plans for residents within the required timeframe and inadequate food handling practices that could lead to cross-contamination. These deficiencies were corrected between May 17, 2022, and June 16, 2022. The Life Safety Code violations involved fire safety concerns, including a lack of annual fire marshal inspections and documentation, insufficient training on fire extinguisher use, and inadequate maintenance records for these devices. All Life Safety Code issues were resolved by January 19, 2023.
