Visit Reason
The inspection was conducted as a renewal visit to review compliance with licensing requirements at Heritage Ridge Senior Living at Johnstown.
Findings
The facility was found to have multiple deficiencies related to resident contracts, staff qualifications, training, medical evaluations, medication storage, and resident assessments. The submitted plan of correction was accepted and fully implemented by early February 2026.

The resident-home contracts for some residents were not signed by the residents as required.
The resident-home contract did not include a fee schedule of actual amounts charged for available services.
The resident-home contract did not explain annual assessment, medical evaluation, and support plan requirements when a higher level of care is needed.
Two direct care staff members lacked verification of a high school diploma, GED, or active registry status on the Pennsylvania nurse aide registry.
During overnight hours, two staff members present were not trained in obstructed airway techniques and CPR, despite 45 residents being present.
Two direct care staff members did not receive the required 12 hours of annual training in 2024.
Some resident medical evaluations were not completed on an annual basis as required.
A prescribed PRN medication was not available in the home at the time of inspection.
A resident was admitted without a completed preadmission screening form within 30 days prior to admission.
Some resident assessments were not completed and signed within 15 days after admission as required.

Visit Reason
The inspection was conducted as a renewal inspection of the facility's license.
Findings
The inspection found multiple deficiencies related to staff orientation, medical evaluations, fire drills, self-administration assessments, and insulin injection training. The facility submitted a plan of correction which was determined to be fully implemented.

Staff person did not receive orientation on fire safety and emergency preparedness topics on the first day of work.
Staff person did not complete training on resident rights, mandatory reporting of abuse and neglect, emergency medical plan, and reporting of reportable incidents within 40 scheduled working hours.

Resident medical evaluations did not include required information such as height, weight, temperature, immunizations, allergies, and physician's license number.
Resident #2 self-administers medication but has not been assessed by a qualified medical professional regarding ability to self-administer and need for reminders.
