Visit Reason
The inspection was conducted on or about August 1, 2018, following a survey by Tennessee Department of Health surveyors to investigate compliance with state laws and regulations at The Glenmary at Evergreen, an assisted-care living facility.

Findings
Surveyors observed serious violations related to the sanitization of dishes and utensils, including the dishwasher being used without sanitizing chemicals and difficulty verifying proper sterilization temperature. A revisit survey on August 31, 2018, found the facility failed to properly implement its Plan of Correction and lacked documentation of staff training on sanitizing procedures.
