The Department of Social Services Community Care Licensing Division conducted an unannounced inspection of OAKMONT OF MOORPARK on 02 March 2023 after a complaint filed 31 January 2023. Evaluator Kasandra Lopez met with Administrator Ronda Wilkin, interviewed four staff members and six residents, and reviewed facility records from late October 2022 through early February 2023. The facility, with a capacity of 112 and a census of 51, is located at 13960 Peach Hill Drive in Moorpark, California.
The inspection substantiated the allegation that the facility is in disrepair. The handicap push‑button at the front entrance and the corresponding internal button were non‑operational, and three fireplaces in common areas were out of service. An ongoing leak from the kitchen to the basement was observed, and the elevator required frequent resets due to power‑related issues. Parts for the push‑button and fireplaces have been on back‑order since November 2022, and the facility’s maintenance director confirmed pending repairs. While the evaluator noted the elevator was being addressed, the combination of non‑functional accessibility devices, heating and fire safety equipment, and unresolved plumbing defects led to a formal citation for disrepair.
All other allegations—an inadequate emergency disaster plan, pest control failures, insufficient food variety, malfunctioning signal system, unsafe parking pathways, inadequate transportation, and poorly communicated outing cancellations—were found unsubstantiated. The evaluator observed evacuation chairs, quarterly drills, and an up‑to‑date emergency plan on file, confirming compliance with Health and Safety Code requirements. Pest inspections revealed no evidence of rodents, and resident interviews reported no concerns about food options or communication systems. Despite the single bus driver limitation, transportation was deemed sufficient, and residents had no reported issues with outing notifications. The report was signed 02 March 2023 and will remain publicly available for three years.
