Inspection Report Summary for Leisure Vale Assisted Living

LEISURE GROVE, LLC, located at 413 E. Cypress Street in Glendale, CA, was the subject of a series of Community Care Licensing Division (CCLD) evaluations from November 2023 through April 2025. The facility operates as a residential care facility for the elderly (RCFE) with a capacity of 199 residents, and it underwent a change‑in‑ownership (CHOW) process during the period covered by the reports. Administrators reported over the period include Aaron Khodorkovsky, Maya Mnoyan, and Angela Smith.

The November 30 2023 visit represented the completion of the COMP II portion of the CHOW process. Administrator Khodorkovsky and the applicant confirmed understanding of licensing laws, admission policies, staffing, emergency procedures, and reporting requirements. The evaluation was closed without noted deficiencies. A month later, on February 22 2024, an unannounced pre‑licensing inspection was conducted. Licensing Program Analysts observed that the building, kitchen, bathrooms, medication storage, and common areas met all applicable standards; the facility’s emergency plans and fire safety equipment were current. The inspection closed with a statement that no deficiencies were found and that pre‑licensing had been successfully completed.

A significant compliance issue emerged during the October 25 2024 case‑management visit. Executive Director Angela Smith reported that staff member S1 had allegedly engaged in financial abuse of multiple residents, with several residents filing police reports. The review of facility records revealed that an incident report for the first identified case was not filed until October 24, violating the 24‑hour reporting requirement of the Code of Regulations. The evaluator noted current occupancy at 169 residents, the distribution of non‑ambulatory and bedridden residents across floors, and the existence of a hospice waiver for 30 residents. The visit found no new deficiencies in physical plant or health‑and‑safety areas, indicating that the facility has maintained compliance with licensing standards since the financial‑abuse incident. The pattern of visits shows that while LEISURE GROVE has met most regulatory requirements, it must remain vigilant about incident reporting and resident protection to sustain its license.
