Surveyors conducted an annual survey at Cades Center of Humboldt on or about March 12, 2019, to assess compliance with assisted-care living facility regulations.

Findings
The facility failed to ensure the registered dietician signed off on the dietary manual and monitored dietary services quarterly. Additionally, multiple residents' files lacked timely written assessments and documentation of involvement of residents or their legal representatives in care plan development and updates.
