Visit Reason
Unannounced routine inspection to review compliance with nursing care facility regulations.
Findings
The inspection found 7 rule noncompliances related to various regulatory requirements including employee identification badges, provider duties to protect clients, policy and procedure training, and quality assurance. No immediate corrections were noted during the inspection.

R432-150-1-4: Identification badges were not worn by all employees providing direct care or volunteers as required.
R380-80-4: The provider failed to fully protect clients from abuse, neglect, exploitation, and mistreatment.
R380-80-5: Provider code of conduct was not fully implemented to protect clients from abuse, neglect, harm, exploitation, mistreatment, fraud, and other compromising actions.
R432-40-3: Employees responsible for assessing, documenting, and reporting had not completed required online training regarding Rule R432-40.
R432-150-4(2)(a-f): The licensee did not provide all required therapies as ordered by licensed practitioners in the resident's care plan.
