Visit Reason
A survey of the Oneida Nursing & Rehab Center was conducted on or about September 22, 2023, resulting in cited deficiencies related to medication administration and medical record maintenance.
Findings
The facility was found deficient for failure to properly supervise and evaluate nursing care, failure to administer prescribed medications to multiple residents on several occasions, failure to notify physicians of withheld medications, and failure to document rationale for medication omissions.

Visit Reason
The inspection was conducted to review compliance with fire and life safety code regulations for an assisted care living facility, specifically regarding fire drill requirements.
Findings
The surveyor found that the facility conducted in-service training for fire drills but failed to conduct the required fire drills for multiple shifts and quarters in 2022 and 2023, including during sleeping hours. The facility admitted these findings.
