Visit Reason
The inspection was the annual survey of Parkdale Health and Rehab to assess compliance with federal and state regulations, including resident care, safety, and facility operations.

Findings
The survey identified multiple deficiencies related to resident care planning, accuracy of assessments, comprehensive care plans, quality of care, treatment of wounds and pressure ulcers, treatment of psych concerns, unnecessary medications, medication errors, dental services, food safety, emergency preparedness, and infection control including COVID-19 protocols.
