Visit Reason
Unannounced routine inspection to review compliance with nursing care facility regulations.

Findings
The inspection found 5 rule noncompliances related to various regulatory requirements including medication administration, resident rights, and facility maintenance. Technical assistance was provided for several areas.

Deficiencies (5)
R432-150-13(5)(a-j) Medication administration deficiencies include failure to administer medications according to professional and ethical practice and facility policies, and improper use of chemical and physical restraints.
R432-150-14(1)(a-c) The licensee failed to ensure each resident is provided necessary care and services to maintain the highest practicable physical, mental, and psychosocial well-being.
R432-150-23(4)(a-r) The licensee did not maintain complete and accurate medical records including assessments, care plans, progress notes, and informed consents.
R432-150-26(2)(a-r) Facility maintenance deficiencies include failure to test, calibrate, and maintain equipment, maintain safe environment, and implement a written maintenance program.
R432-150-17(5)(a-d) Medication storage deficiencies include failure to store drugs in locked compartments with proper temperature controls and separation of medications from non-medication materials.
