One violation was established related to inadequate supervision of Resident A resulting in multiple falls. The allegation that Resident A was not charged according to his contract was not established.

R 325.1921 Governing bodies, administrators, and supervisors: the facility failed to provide adequate supervision for Resident A, who had four falls due to poor safety awareness and inability to provide one-on-one care. The use of a tilt back wheelchair seat was considered a form of restraint and not an appropriate supervision method.

R 325.1953 Menus: The menu posted in the assisted living dining room was not updated to reflect the current week and instead listed meals for the week of 9/3/23-9/9/23.
