Additionally, a complaint investigation in February 2023 determined immediate jeopardy conditions related to infection control failures during simultaneous norovirus and COVID-19 outbreaks affecting numerous residents. Findings also included improper use of protective equipment, inadequate isolation measures, and staff noncompliance with infection control procedures.

Other inspections also documented medication error rates above accepted thresholds, concerns regarding medication storage and documentation, and failures to notify physicians when medications were refused by residents. A more recent inspection in March 2025 also identified issues involving privacy protections, PICC line care competencies, and food temperature monitoring. However, a follow-up survey completed in April 2025 confirmed that corrective actions had been implemented for all cited deficiencies.

On the other hand, quality outcome measures indicate that some resident care metrics, including falls with major injury, functional decline, and hospitalizations, remain above Rhode Island averages. However, the community has not received fines or license suspensions, and regulatory follow-up reports show ongoing efforts to improve compliance and resident safety practices.
