Visit Reason
A Recertification Survey was conducted from 11/27/2023 to 11/30/2023 to assess compliance with 42 CFR Part 483, Subpart B, for Long Term Care Facilities. A Life Safety Code survey was also conducted on 12/05/2023.

Findings
The facility was found noncompliant with requirements related to Activities of Daily Living (ADL) care, specifically failure to provide adequate fingernail care for Resident #15. Additionally, the facility failed to provide documentation of testing for Patient-Care Related Electrical Equipment (PCREE) as required by NFPA 99.

F677: The facility failed to provide adequate fingernail care for Resident #15, who had long and dirty fingernails observed on multiple days. Staff did not document refusals of care and did not consistently provide nail care as required by the care plan.
K921: The facility failed to provide documentation of testing and maintenance for Patient-Care Related Electrical Equipment (PCREE) in accordance with NFPA 99. This deficiency affected 3 of 3 smoke compartments.
