Visit Reason
The inspection was a Comparative Federal Monitoring Survey conducted following a State Agency Annual Survey to assess compliance with federal regulations for long term care facilities.
Findings
The facility was found noncompliant with emergency preparedness communication plan requirements and life safety sprinkler system installation standards. Deficiencies affected smoke compartments, staff, and residents, but no residents were harmed.
