Incidents reported during the year were handled promptly and in accordance with regulations. On 26 December 2023 a staff member discovered a resident (R1) with a pair of scissors and a minor injury in the resident’s apartment. The facility removed potentially harmful items, provided a private 24‑hour companion, and conducted staff in‑service training. The resident, diagnosed with mild cognitive impairment, had no prior self‑harm risk and is reported to be doing well. In a 20 September 2024 case‑management visit, an unsealed cabinet in a resident’s room contained over‑the‑counter medications and acetone alcohol. The facility immediately secured the items and documented the correction. Each incident review involved thorough record examination, direct resident interaction, and corrective action before the LPA completed the inspection.

The facility’s readiness for licensing was confirmed in the pre‑licensing report, with final approval pending at the Central Application Bureau.
