Savant of Woodland Hills (Facility #195850546), a 740‑type residential care facility in Woodland Hills, California, was the subject of three separate Community Care Licensing Division (CCLD) evaluations in 2024‑2025. The reports include an unannounced complaint investigation, an office‑announced Component II evaluation, and a pre‑licensing site visit as part of a change‑of‑ownership application.

During the 06/13/2025 complaint investigation, LPA Zabel Chochian reviewed allegations that staff had moved a resident to the memory‑care unit without documenting a change in the resident’s condition or obtaining prior consent. Interviews with the resident, family, and staff, combined with a review of medical and admission records, confirmed that the resident had shown cognitive decline and was moved on 04/09/2025 after family consent and a signed admission agreement. The investigation concluded the allegations were unsubstantiated, and the report was signed by the evaluator and facility representative.

The 12/04/2024 Component II evaluation, conducted via telephone, confirmed that administrator Sidney Kevan and owner Adam Zenou understood the Title 22 licensing requirements, including admission policies, staffing, emergency preparedness, and complaint reporting.
