Visit Reason
Follow-up inspection to verify correction of previously cited deficiencies related to licensing laws and regulations for an Assisted Living Facility.
Findings
The follow-up inspection found no deficiencies and confirmed that the facility meets Assisted Living Facility licensing requirements. Previously cited deficiencies related to tuberculosis testing and staff training were corrected.

Visit Reason
The Department of Social and Health Services conducted a follow-up visit to The Park at Belle Harbour to assess correction of previously cited deficiencies related to tuberculosis testing and staff training.
Findings
The facility failed to complete required tuberculosis two-step skin testing for three staff members and failed to ensure one staff member completed required training, placing all 31 residents at risk. These deficiencies were uncorrected from a prior citation dated April 2, 2025.
