Lakes, The, a residential care facility for the elderly in Banning, California, was the focus of several Department of Social Services inspections and complaint investigations in late 2024 and early 2025. Administrator Lori Matsushita oversaw the site during a September 2024 change‑of‑ownership application and a November 2024 competency review. The facility’s 276‑bed capacity was reported to be occupied by 99 residents during the pre‑licensing visit, with census figures ranging from 100 to 115 in subsequent visits.

The December 6, 2024 pre‑licensing evaluation by Licensing Program Analyst Sarina Ramirez found the physical plant, staffing, and health‑and‑safety arrangements in compliance with California Code of Regulations Title 22. Bedrooms, bathrooms, and common areas met required standards, and the facility had adequate supplies, a functional first‑aid kit, and posted required signage. Ramirez concluded that the facility was ready for licensure and no corrective actions were required.

In February 2025, an unannounced complaint investigation substantiated allegations that staff had repeatedly stolen residents’ personal property. The investigation identified a deficiency under CCR 87217(b) for safeguarding residents’ valuables and required the administrator to submit a plan of corrections by February 14, 2025. Two subsequent complaints—one involving a resident being pushed by an unknown person (March 12, 2025) and another unsubstantiated claim received on April 14, 2025—were investigated and found not to be supported by sufficient evidence, and no further corrective action was mandated. Overall, the facility demonstrated adequate compliance during its licensing review but faced a significant property‑safety issue that is now under corrective monitoring.
