In September 2022, the California Department of Social Services Community Care Licensing Division conducted an unannounced investigation at Variel of Woodland Hills, the 740‑capacity facility located at 6233 Variel Ave. Evaluator Ashley Smith interviewed staff and a resident (Resident #1) and reviewed internal documents. The investigation confirmed that staff had inappropriately pulled Resident #1 by the wrists during a transfer, causing bruising on the forearms, and that the facility failed to submit an Unusual Incident Report to the Department.
