Inspection Report Summary for The Vincent

Oakmont of San Rafael (Facility #216804010) underwent a series of licensing and compliance visits from October 2021 through June 2022. The first two visits were Component II telephone reviews, conducted on 10 December 2021 by Administrator Melon Rivera and on 2 April 2022 by Administrator Jill Libhart, following an administrative change. Both reviews confirmed the facility’s understanding of Title 22 regulations, staffing qualifications, and program policies, and noted that no residents were in care at the time of each call.

In March 2022, an unannounced pre‑licensing inspection verified that Oakmont met all requirements for a 126‑bed, non‑ambulatory residence with eight bedridden residents, effective 13 January 2022. Inspectors observed a clean, temperature‑controlled environment, unobstructed exits, and properly stored perishable and non‑perishable foods. The Memory Care unit featured appropriate water temperatures, non‑slip flooring, handrails, and functional egress. Locked medication, laundry, and housekeeping rooms were noted, and the lounge/bar with alcohol was secured outside operating hours. Fire extinguishers had last been serviced in March 2021, with a scheduled service in March 2022. The facility’s emergency disaster plan, first‑aid and CPR certificates, and generator were all in order, and Component III was completed with the administrator.

A post‑licensing inspection on 17 June 2022 echoed the earlier findings. The facility remained clean, with exits unobstructed and food storage compliant. Water temperatures and safety features in both the Memory Care unit and the general unit rooms were within acceptable ranges. Fire extinguishers had been serviced in April 2022. The lounge/bar remained locked during non‑operating hours, while the exercise room stayed open and the salon locked when not in use. No deficiencies were reported, and the inspector advised routine communication with County Public Health for any COVID‑19 or health concerns. Across all visits, Oakmont demonstrated consistent compliance with California Department of Social Services standards in care, safety, and administrative oversight.
